Dr. Hays is one of our great medical providers and currently oversees our Frisco clinic, which has been open since January 2019. He received his B.S. from Northeast Louisiana University in 1980 and later completed and obtained his Medical Degree from Louisiana State University School of Medicine in 1984. Dr. Hays did his residency training at LSU Medical Center in Shreveport which he completed in 1988. He also has over 30 years of experience as a staff physician, emergency staff physician, and as a medical director at various urgent care clinics including CareNow. Dr. Hays joined the DFW Neuropathy team in October 2018, and we are truly honored to have him on board.
Dr. Hays has been married for nearly 40 years, has two children and loves spending time with his family. He’s an avid member of his church and has been a Worship Leader since 1979 and also enjoys the occasional game of golf when he has the time.
A lifelong Texan, Dr. Roberts was born in Tyler, Texas and graduated magna cum laude from Austin College with a major in Biology in 1995. After attending medical school at the University of Texas Health Science Center San Antonio, she moved to Dallas for her Family Medicine Residency at Baylor Garland in 1999. She has practiced Family Medicine in Carrollton, Plano, and Dallas since 2002 and joined DFW Neuropathy in 2019.
She and her husband Matt have two teenage daughters, two dogs, two guinea pigs, and a turtle. She enjoys yoga, running, barre classes, reading, and watching her daughters dance.
Dr. Flowers is a family medicine physician who was born in Oklahoma and raised in the Bay Area of Northern California. She received her BS in Animal Physiology from the University of California, Davis and her Medical Doctorate from the University of Arkansas for Medical Sciences. Dr. Flowers did her residency training in Family Medicine at John Peter Smith Hospital in Fort Worth, TX. She has experience in providing primary, urgent, geriatric, hospice, and occupational care and has also worked in multiple healthcare settings including military units nationwide. She joined DFW Neuropathy in 2018 and enjoys treating and restoring the lives of those with Peripheral Neuropathy. Dr. Flowers values teaching patients preventative measures to improve their quality of life.
Outside of her clinical duties, Dr. Flowers is also a public speaker who has done presentations on various topics concerning all age groups. She also works diligently to build stronger communities and volunteers in various projects. Furthermore, in 2018 Dr. Flowers proudly became a best-selling author on Amazon.
Jennifer McIntosh, FNP-C, is a nationally board certified Nurse Practitioner who joined DFW Neuropathy in May 2016. Jennifer (Jenni) started her nursing career in 1994 as a Licensed Practical Nurse and went on to obtain her undergraduate degree from Louisiana Tech University. Her years of nursing experience include medical, surgical, acute care, and critical care. Jenni moved to Texas in 2010 and went on to become a school nurse while her children were in grade school. Motivated to be a voice of change and a patient advocate, Jennifer went on to pursue her Master of Science degree from Indiana State University. She is board certified as a Family Nurse Practitioner through the American Academy of Nurse Practitioners.
Jenni’s professional training includes family practice, weight management and urology.
Her years of training and experience in the nursing profession have shaped her to become the provider she is today. Jenni’s personal philosophy of nursing is rooted in a commitment to public service and undeniable desire to help those in need. Nursing is more than treating an illness; rather it is focused on delivering quality patient care that is individualized to the needs of each patient. Jennifer wants to focus on her patients more holistically by incorporating nutrition, disease prevention, and education into her practice which led her to find DFW Neuropathy.
Jenni loves spending time with her husband, three grown children, and her bulldog, Harley. She enjoys exercising, running stadiums, playing softball and volleyball in her spare time. DFW Neuropathy is proud to have Jennifer as part of the team and family.
Dr. Alavi is the founder, President and Chief Executive Officer of DFW Neuropathy. He currently specializes in the treatment of Peripheral Neuropathy utilizing The Combined Electro-analgesia Treatment and various combined non-invasive and effective therapeutic modalities for this common and debilitating condition. Dr. Alavi received his Bachelor of Science Degree in Cell and Molecular Biology and Chemistry from The University of Nevada Las Vegas, with a postgraduate degree in Microbiology and Virology. After his postgraduate degree Dr. Alavi completed and obtained his medical degree at Touro University College of Osteopathic Medicine in California. Dr. Alavi attended his residency and completed his Cardiology Track-Internal Medicine training at The University of North Texas Health Science Center in Fort Worth, Texas. Dr. Alavi has focused and worked in various fields of medicine, such as, General Primary Care, Cardiology, Hospital Medicine including ICU and Critical Care, Hospice and Palliative Care, and finally Peripheral Neuropathy. He has previously been a leading investigator for various clinical trials on Peripheral Neuropathy and currently serves as an advisor for various research, educational, and charitable institutions. Dr. Alavi is a firm believer in Evidence Based Medicine, holistic and all-inclusive patient care, and is compassionate about truly caring for his patients. His hobbies include, traveling, reading, and being involved in humanitarian projects for those most in need through various organization, churches, and charitable foundations.
Dionna Dalzell, APRN-C, is a nationally Board-Certified Nurse Practitioner who joined the DFW Neuropathy family in July 2020. Dionna began her career in high school by working as a certified nursing assistant in a nursing home. Since that time, she has accumulated years of nursing experience and education in both larger university hospitals and small outpatient clinics. Her scope of practice encompasses health promotion, disease prevention, and management of common and complex health care problems. Dionna most recently worked in a neurosurgery specialty clinic and assisting in the operating room. She has family practice and pain management experience dealing with chronic pain with an emphasis on treating low back and neck pain, radiculopathy, and Peripheral Neuropathy.
Dionna earned her Master of Science degree from Creighton University in Omaha, Nebraska in May of 2005. During her career she had the privilege to work at a state of the art medical and teaching trauma hospital on a float team that was utilized in all areas of the hospital. Dionna’s undergraduate degree, a Bachelor of Science in Nursing was completed at Methodist College of Nursing and Allied health in Omaha, Nebraska. Dionna is a member of the American Association of Nurse Practitioners and a member of the Texas Nurse Practitioners.
From starting out as a nursing assistant to becoming a provider, her personal goal has always been centered on quality of life. “I want to help people with holistic practices to nurture wholeness and healing by respecting each patient’s unique physical, mental, emotional, spiritual, and environmental strengths and challenges by honoring each patient’s values, health beliefs, and health experience.” She believes a positive healing experiences starts by taking the time to listen and understand each person’s particular needs. This mission has led her to DFW Neuropathy. Dionna is committed to providing the highest quality healthcare in a professional environment that puts the patient first.
Dionna and her husband are extremely proud parents of 3 grown children and 1 grandchild. Dionna also enjoys time with her family, reading, hiking, and camping.
